A registered nurse is always on call, 24 hours, to answer questions and to provide assistance and support when needed. We will provide care to every eligible patient in our service area, regardless of medical conditions, lifestyle, religious, cultural or ethnic background. We believe all patients who are eligible for hospice should receive hospice. After all this is the time When Life Matters Most...

Duties:

Nurse Liaison:

• Responsible for evaluation of all referrals to Hospice including, but not limited to patient/family assessment, medical record review, and evaluating for Hospice appropriateness, and discharge planning.
• Assists with contacts/educational offerings to physicians and their staff.
• Required: Registered Nurse with Bachelors in Nursing.
• Preferred: A minimum of five years hospice experience.
• Excellent oral and written communication skills.
• Experience as a discharge planner, case manager, and/or educator desired.
• Must have current RN license and a driver's license.
